Description
This certificate program prepares students to enter the workforce as Community Health Navigators or to continue their credentialing in a health-related field in Care Coordination. Students will develop critical skills to become advocates for the population(s) they serve and support. Students will gain an understanding of community health and the healthcare system. They will learn skills in health information technology, as well as specific communication and motivational interviewing techniques. Students will gain knowledge of how to use these skills to provide guidance to patients and support patient behaviors that promote value-based outcomes via reduction in chronic disease, hospital admissions, and healthcare costs. When implemented by Care Coordinators or Community Health Navigators, skills learned as part of this certificate will facilitate more efficient use of practitioner time, creating cost savings within the healthcare field. Within this certificate program, students can earn the Multicultural Diversity World Health Perspective microcredential and utilize coursework toward eligibility for certification in Health and Wellness Coaching. Courses in this Certificate can be applied toward course requirements in the MCC Health Studies AS degree program.
